Intake Form - Store per customer

Currently has to be completed each time a customer makes a booking which is far from ideal. Surely this can be done once and stored in the account? It can be generic form which covers all treatments. Can you please assist with this because I can see us losing bookings if customers need to fill it out each time. We have customers who want to book at least once per month. Thanks Kevin
